D&D General How would you make this ruling? Vortex Warp

Gut bacteria may not have a stat block, but by definition, it is a monster, for what that's worth.

This would be a complete house ruling, but I'd let the caster make a check with their spellcasting skill. I'd just make it a 15 to start with. Success? Works as they want. Failure? The swallowed character takes some damage from being bumped around and is still swallowed.
Eh, I wouldn't impose any damage on the swallowed creature. All those squishy internal organs surrounding them will cushion the blow. Plus they'll be taking plenty of damage from the stomach acid. No need to punish the PC more for having had the gall to get swallowed (pun intended).

This is the fun kind of ruling that can ruin a game.

Most players are super happy to say "sure the 2nd level spell can teleport away any monster that swallows a 'poor' PC".

And many will say it is true for a grapple or other such holding of a creature.

Many might do it case by case, where the players approve when it somehow benefits them in the game...

Though you might see the crack as soon as this is used on a player. As soon as a weak kobold sorcerer frees a foe from a PCs grip, they players will be quick to cry "no fair" or "the spell should not work that way!" or the classic "this game is not consistent!"

This is why I do recommend having a DM make and enforce the ruling that is best for the game. But that is just my thoughts....
